IMPORTANT NOTE WHEN USING THIS FORM
De Anza College Course Information:
List the De Anza course(s) that you want to take, which require prerequisites already fulfilled by you at another institution.


Exceptions to Using this Form
Some departments require additional information
or a different form to be completed. Please follow the links if your course department is listed.

Adobe Reader 8.0 or higher is required to save and/or e-mail your completed form. Download Adobe Reader.


Step 2 Gather Proof of Prerequisite Coursework

You must provide the following documents.

(A) Catalog description from prior college for course(s) that you want considered for prerequisite clearance AND

(B) College transcript with final grades for courses from prior colleges that you want considered for prerequisite clearance.

For your transcript, include either an Official Transcript in a sealed envelope (if you plan to attend De Anza for two quarters or more)  OR an Unofficial Transcript (if you plan to attend De Anza for only one quarter).

Foothill students do not need to attach a Foothill transcript or course description.


Step 3 Submit Completed Form with Required Documentation

Assessment Center Drop Box LocationSubmit your request at least 3 to 4 weeks before your registration date.

  • Follow instructions on the form for submitting your request based on your transcript type and if you are on campus.

  • The on-campus Prerequsite Drop Box is located near the south entrance of Student and Community Services Building near Admissions. See map.

Step 4 Check for E-mail Notification

You will receive an e-mail regarding the results of your request. Allow 3 to 4 weeks for a response after your paperwork is submitted. Please update your personal e-mail information in MyPortal and check your e-mail often.
