How to Apply for DSPS Services

You Must First Register as a De Anza College Student

After becoming a De Anza College student, return to this page with your College Wide Identification (CWID) number.

DSPS Application Process for New or Returning* Students

During the online DSPS Application process, you will be asked to upload your most recent disability related documents**.  If your documentation is not in a digital format or if you are unable to scan and upload these documents you may provide copies by Fax or in person at:

The Disability Support Services (DSS) office located on the De Anza College campus in the Student Community Services Building, Suite 141 - FAX # 408-864-5492 – Office# 408-864-8753


**Documentation examples:

  • A De Anza College Disability Verification Form signed by the appropriate medical professional.  This form is available at the DSS office or online at: DSPS Disability Verification Form
  • A Psycho-Educational Assessment from a Psychologist
  • A Regional Center IPP
  • An Individualized Education Plan (IEP) / 504 Plan

*Returning students are those who have not been enrolled at De Anza for at least one year.

 Steps for Accessing the DSPS Application

Go to the De Anza College Portal and log in using your CWID number and password.
Select the "Student" tab
Find the "Clockwork Student Portlet" box and select "ClockWork De Anza Disability Progs & Svcs"
On the ClockWork main menu, click on the DSPS Application icon
Complete all SIX sections of the DSPS Application.
Answer the security question and select "Submit".

 What happens next?

Upon completing the DSPS Application process and clicking the "Submit" button, a DSPS staff member will contact you to set-up an appointment with a DSPS counselor.
