How to Apply for Deaf Services
Seven Steps for New or Returning Students
- Take the De Anza College Disability Verification Form to your audiologist or doctor.
- Turn in your Disability Verification Form and audiogram to DSS.
- Schedule a DSS Intake-Registration Appointment.
- Attend the Intake-Registration Appointment.
- Register for classes with your DSS Counselor.
- Be ready to pay college registration fees at the Appointment.
- Turn in a Deaf Services pink "Services Request Form" and meet with the Deaf Services Supervisor.

Step 1

Take the De Anza College Disability Verification Form to your audiologist or doctor |
New Students
- Download and print the De Anza Disability Verification Packet
- Includes Disability Verification form.
- If you want it mailed to you, contact Disability Support Services (DSS) at 408. 864.8753, TTY 408.864.8748, Email: dss@deanza.edu .
Returning students (away from De Anza for one year or more)
- Contact the DSS Secretary at 408. 864.8753, TTY 408.864.8748, or Email: dss@deanza.edu
- to find out if you need a new Disability Verification Form or audiogram
- to make an appointment with a counselor to review and update the Student Education Contract.
Both new and returning students
- Take the Disability Verification form to your doctor or audiologist to have it filled out and signed.

Step 2
|
Turn in your Disability Verification Form and audiogram to DSS.
|
- Make a copy of your audiogram.
- It has to be within the last three years.
- If it is more than 3 years ago, have a new audiogram.
- Turn in the completed and signed Disability Verification Form and the audiogram to:

Step 3
|
Schedule a DSS Intake-Registration Appointment.
|
- After your Verification Form and audiogram are received, DSS will call or email you to schedule an Intake-Registration appointment with a DSS counselor.
- Request an interpreter or a captioner if you want one for the appointment.

Step 4
|
Attend the Intake-Registration Appointment.
|
- You will meet with the DSS counselor and the Deaf Services Supervisor. The meeting lasts about one hour.
- You and the counselor will talk about:
- your goals,
- your past education,
- your communication preferences,
- the De Anza classes or programs to meet your goals,
- the services you will need,
- finishing the De Anza College Application if you haven't done it on-line already.
- You might have to take the English or Math Placement tests to learn your skill levels, The counselor will tell you if you need do after you tell about your past education and your goals.
- If you have finished classes at another college, bring your transcript or grade reports.
- The Counselor will explain how to take the Placement tests.

Step 5
|
Register for classes with your DSS Counselor.
|
- You may register for your classes at the Intake-Registration Appointment,
- If there is enough time and
- If you don't need to take the placement tests or already took them
OR
- You might have another appointment with the counselor on another day to register for your classes.

Step 6
|
Be ready to pay college registration fees at the Appointment.
|
- When you register for classes, you have to pay your college fees.
- You can pay by check or credit card.
- The counselor can tell you about financial aid programs and help you apply.
- The college will give you a refund if you qualify for financial aid after you enroll.
- If you are a Department of Vocational Rehabilitation client and they will pay your fees,
tell the counselor.

Step 7
|
Turn in a Deaf Services pink "Services Request Form" and meet with the Deaf Services Supervisor.

- To get your classroom services, you will
- fill out and turn in a class "Services Request" form, available in DSS or the Deaf Services Office and
- make an appointment to meet with the Deaf Services Supervisor.

- the college guidelines for receiving services
- any other questions you have about Deaf Services
