Counselor's Recommendation Documents

  • College Official's Form (for the common application - private colleges/universities)
  • Letters of Recommendation for College/University, Scholarships, Employment
  • Reference Check
  • Appeal Letters for the Admissions to the Colleges/Universities

Students who require any of the above documents must request this at least 10 school days prior to the recommendation due date.  Last minute request will not be accommodated. 

To request a recommendation document, students must provide the following:

  • Completed Student Information Sheet
  • Background information regarding your request (i.e. Scholarship Info, Job Description, etc.)
  • Due Date of the recommendation document 

For those who need to complete the Common Application: College Official's Form, you must sign the "Important Privacy Note" (Red Box) before submitting your request.  Once your form has been completed by a counselor, you must bring your form to the Office of Student Development (located in lower level of the Campus Center) for Dean Michele LeBleu-Burns signature.  She will complete the section regarding your disciplinary record at De Anza College.
